Outpatient Program at DCHV
Nestled in the heart of our community, the Delaware Center for Homeless Veterans' Outpatient Program is a beacon of hope and support. Our program is committed to delivering patient-centered care, focusing on adults aged 18 and older, including young adults, middle-aged, and older adults. We specialize in offering comprehensive services to those struggling with co-occurring disorders, mental health, and substance abuse issues. Our services include outreach and case management, aiming to assist in establishing psychiatric, psychosocial, and available resources for our clients. We are dedicated to empowering patients in their recovery journey, enhancing their overall quality of life, and helping them maintain their independence and daily routines.
